Sliding Fee Discount Program

At United Citizens Foundation (UCF) and Miracle Minds Therapy, we believe that no one should ever be denied care because of an inability to pay.
Our Sliding Fee Discount Program makes behavioral-health services affordable for everyone—whether you have insurance, are under-insured, or have no coverage at all.

How It Works

The sliding fee program adjusts the cost of services based on your household size and total family income in relation to federal poverty guidelines.
This ensures that individuals and families with limited income can still receive high-quality care at a reduced cost—or in some cases, at no cost.

Eligibility

  • Available to all clients, regardless of insurance status.

  • Discounts are based on family size and income (as verified through a brief application).

  • Proof of income may include recent pay stubs, tax returns, or a self-attestation form if documentation is unavailable.

  • Eligibility must be renewed once per year.

How to Apply

  1. Complete a Sliding Fee Application (available in English and Spanish).

  2. Submit proof of income or a self-attestation form.

  3. Our staff will determine your discount level using the Sliding Fee Scale shown below.

If your household income is at or below 200% of the federal poverty level, you may qualify for a discount.
Those with income below 100% of the poverty level will not be charged a fee and will be screened for Medicaid eligibility.
